Revised HSG Cardiovascular Disease Risk Calculator

Introduction:

This calculator computes the 10-year Coronary Artery Disease (CAD) risk based on the recalibrated Singapore-modified Framingham Risk Score (SG-FRS-2023) for the general population. In addition, it reminds users to adopt target HbA1c based on individual patient's context and generates target levels for LDL cholesterol and BP for patients at various levels of cardiovascular risk.

Note: SG-FRS-2023 is validated only for ages 20–79. For patients aged ≥80, Questions 3–8 are not required.
Please proceed to Questions 9–13.

1. Please select patient's GENDER
2. Please select patient's AGE (years)
3. Please select patient's ETHNICITY: Note: As there is insufficient data for other ethnic minorities, the 10-year Coronary Artery Disease risk score for the lowest risk group (i.e. Chinese) applies for these individuals.
4. Is patient a cigarette SMOKER?
5. Please select patient's HDL Cholesterol range - mmol/L (mg/dl)
6. Please select patient's Total Cholesterol (TC) range - mmol/L (mg/dl)
7. Please select patient's SYSTOLIC BP range - mmHg
8. Is the patient on antihypertensives?
9. Does the patient have a history of Acute Coronary Syndrome (ACS)?
10. Does the patient have a history of atherosclerotic cardiovascular disease (ASCVD), other than ACS? Note: E.g., History of other stable ischaemic heart disease, ischaemic stroke, transient ischaemic attack, peripheral artery disease, abdominal aortic aneurysm, post coronary artery bypass graft, post percutaneous coronary intervention.
11. Does patient have familial hypercholesterolaemia (FH)? Note: If LDL-cholesterol > 4.9 mmol/L, consider work-up for FH.
If LDL‑C ≥ 5.5 mmol/L, may refer to Genomic Assessment Centre for FH index genetic testing.

Additional cardiovascular risk factor(s) e.g.,subclinical atherosclerosis (if investigated).
12. Does the patient have chronic kidney disease? Note: i.e., eGFR < 60ml/min/1.73㎡ OR
uACR ≥ 3mg/mmol (30mg/g) OR
uPCR >15mg/mmol (150mg/g).
13. Does patient have diabetes mellitus (DM)? Specific risk factors include: (1) Chronic kidney disease; (2) Multiple microvascular complications; (3) Diabetes duration ≥ 10 years; (4) Glycaemic level persistently above target despite optimal treatment.
